Asked for a copy of what I signed, but given this blank one instead.
Following is the initial application filled out to take showers (open only 4 days a week, Mon-Wed-Fri-Sun and then only in the mornings 9 to 1pm)
When one goes to the shower, one must sign up on a clipboard, giving name, birthdate, and last time a doctor was seen.
Only water source and the rules and regulations board.
